Titolo: Attuale ruolo diagnostico e prognostico della des-gamma-carbossi-protrombina nell'epatocarcinoma
Abstract: ITALIANO. Negli ultimi decenni l'HCC ha fatto registrare un notevole incremento di frequenza, tanto da rappresentare fino al 3-6% di tutti i tumori solidi negli USA e in Europa e fino al 20%-40% in Africa e Sud-Est Asiatico. La strategia di screening per HCC attualmente raccomandata per i pazienti con cirrosi epatica comprende la determinazione sierica dell'alfa-fetoproteina (AFP) e una ecografia addominale ogni 6 mesi, allo scopo di rilevare l'HCC in fase precoce. La des-gamma-carbossi-protrombina (DCP)à à ¨ un importante marcatore bioumorale dosabile nel siero dei pazienti con HCC, ma non rilevabile in quello dei soggetti sani. La DCP nel siero ha una sensibilita' compresa fra 48% e 62%, una specificita' compresa fra 81% e 98% , ed una accuratezza diagnostica compresa fra 59% e 84% per la diagnosi di epatocarcinoma in diversi grandi studi caso-controllo. Secondo recenti studi in combinazione con altri biomarcatori, la DCP potrebbe migliorare lo sviluppo di efficaci sistemi prognostici e di staging per HCC. Inoltre la DCP sembra avere un ruolo importante nell'angiogenesi e nella progressione dell'HCC.
ENGLISH. Major points. Acarboxyprothrombin is an abnormal prothrombin induced by the absence of vitamin K-II that is increased in the serum of patients with hepatocellular carcinoma (HCC). In hepatoma cells, genetic alterations, membrane receptors, the inability to uptake labelled low density lipoprotein (LDL), cytoskeletal changes and hepatocyte cytoplasmic transfers involved in vitamin K metabolism could play an important role in producing detectable acarboxyprothrombin serum levels. Serum acarboxyprothrombin was found to have a sensitivity ranging from 48% to 62%, a specificity of 81% to 98%, and a diagnostic accuracy of 59% to 84% for detecting HCC. Plasma acarboxyprothrombin does not correlate with alpha-fetoprotein levels. However, when used together, the acarboxyprothrombin and alpha-fetoprotein assays increase the sensitivity of detecting HCC in more than 85% of patients. The specificity of the acarboxyprothrombin assay appears to be superior to that of alpha-fetoprotein. These biomarkers can complement ultrasound for early HCC detection, but neither acarboxyprothrombin nor alpha-fetoprotein is optimal. For small HCC, a high preoperative acarboxyprothrombin level appears to be indicative for tumour recurrence. Recently, there has been attention given to acarboxyprothrombin because of its role in detecting HCC recurrence after living donor liver transplantation. More recent research has demonstrated that acarboxyprothrombin stimulates human vascular endothelial cell growth and migration. Conclusions. All the data presented above demonstrate the importance of acarboxyprothrombin in formulating a prognosis for patients with HCC.
